If you are an existing Medicare client with our office, soon you will be receiving a Medicare packet with a form where you can update your current prescription drug list and other health details. Please fill out this form and return it as soon as possible. Have this completed form helps us:

Steps to Take Before December 7
Here’s how to make the most of this enrollment season:
1. Complete Your Medicare Packet
- Fill out the drug list update form included in your packet.
- List any new doctors, treatments, or medications since last year.
- Bring the packet to your in-person appointment — this helps us get straight to the comparisons that matter most to you.
2. Review Your Current Coverage
- Think about what worked well and what didn’t this year.
- Consider costs, network changes, and benefits like dental or vision.
- We’ll go through everything together during your appointment.
3. Make Your Appointment
- For those of you who need to make plan changes, or prefer to sit down and discuss next year’s plan options, Medicare conversations are best held in person, and are by appointment only. Please call ahead to make an appointment at a time that works best with your schedule.
- Bring your completed drug form or any additional questions with you so we can review any changes in costs, coverage, or provider networks together.
